چکیده

Introduction: None of the currently available pulmonary artery (PA) catheters allows instantaneous and continuous measurement of pulmonary blood flow. At best, the socalled continuous cardiac output (CO) catheters (and associated software) indicate continuously an averaged value of CO measured over several minutes. A new catheter has been introduced recently, and animal experiments have shown that it allows instantaneous detection of changes in pulmonary blood flow. In this catheter, blood flow is derived from the power required to maintain a temperature gradient between two thermistors that are insulated differently. The technology also allows the clinician to measure CO using the standard thermodilution technique. The aim of the present study was to demonstrate the clinical safety and efficacy of this new PA catheter.

Comparison of esophageal Doppler, pulse contour analysis, and real-time pulmonary artery thermodilution for the continuous measurement of cardiac output.

OBJECTIVE Continuous measurement of cardiac output (CCO) is of great importance in the critically ill. However, pulmonary artery thermodilution has been questioned for possible complications associated with right heart catheterization. Furthermore, measurements are delayed in the continuous mode during rapid hemodynamic changes. A new pulmonary artery catheter CCO device (Aortech, Bellshill, Sc...

[A unique case of pulmonary artery catheter bleeding from the oximetry connection port].

Pulmonary artery catheter is an invasive monitor usually placed in high-risk cardiac surgical patients to optimize the cardiac functions. We present this case of blood oozing from the oximetry connection port of the pulmonary artery catheter that resulted in the inability to monitor continuous cardiac output requiring replacement of the catheter. The cause of this abnormal bleeding was later co...

Cardiac output monitoring: aortic transpulmonary thermodilution and pulse contour analysis agree with standard thermodilution methods in patients undergoing lung transplantation.

PURPOSE The PiCCO System is a relatively new device allowing intermittent cardiac output monitoring by aortic transpulmonary thermodilution technique (Aorta intermittent) and continuous cardiac output monitoring by pulse contour analysis (Aorta continuous). The objective of this study was to assess the level of agreement of Aorta intermittent and Aorta continuous with intermittent (PA intermitt...

Clinical relevance of data from the pulmonary artery catheter

The usefulness of parameters measured using the pulmonary artery catheter has been challenged because no benefit in patient outcome has been observed in clinical trials. However, technological advances have been made, including continuous measurement of cardiac output (CO), mixed venous saturation (SvO2), and right ventricle end-diastolic volume (CEDV) have been made. Pulmonary artery occlusion...

Transpulmonary thermodilution: advantages and limits

BACKGROUND For complex patients in the intensive care unit or in the operating room, many questions regarding their haemodynamic management cannot be answered with simple clinical examination. In particular, arterial pressure allows only a rough estimation of cardiac output. Transpulmonary thermodilution is a technique that provides a full haemodynamic assessment through cardiac output and othe...
